At Proactive Hospice, patients are cared for by an interdisciplinary team of professionals.

Who Is the Hospice Team?

• Medical Director The Hospice Medical Director works alongside the hospice team and the patient’s primary doctor to effectively manage the patient’s discomfort and symptoms.
• Attending Physician The attending Physician refers patients to hospice care while continuing to act as the patient’s primary care physician and collaborating with the hospice team.
• Registered Nurse Registered Nurses specialize in palliative care and pain management. They assist patients in achieving and maintaining comfort and a high quality of life. Additionally, they are responsible for educating family members on how to enhance the care of their loved ones.
• Certified Nursing Assistant Nursing Assistants provide essential services, including bathing, personal care, and comfort care for patients.
• Medical Social WorkerMedical Social Workers offer counseling to individuals and families to address the challenges posed by a terminal illness. They also promote family communication, organize caregiver support, and assist in finding financial and legal resources.
  
• Hospice ChaplainChaplains offer spiritual counseling to individuals of all faiths, assist in planning services, and act as a link between families and clergy in the community.
  
• Bereavement CounselorBereavement Counselors provide various services to support the family of the patient in understanding and coping with grief.
  
• Hospice VolunteerHospice Volunteers are compassionate individuals dedicated to supporting patients and their families by providing respite care.
